Sub-Total of your order before any tax is added). 

Rates below are base rates for jewelry and lighter items.  Heavier items, such as plaster
sculpture and bronzes, will different shipping and handling charges (each), which are noted on the web page
featuring those items. If you are ordering a combination of lighter and heavier items, please use the
rates below for your lighter items, then add the s/h from the web page for your heavier items.

IMPORTANT NOTICE TO CUSTOMERS SHIPPING OUTSIDE THE UNITED STATES OF AMERICA:

Please be aware that all items we sell are shipped from the United States of America to your destination. This means that if the destination for your order is outside the United States, as far as YOUR government is concerned, you are IMPORTING goods from a foreign country, and those goods may be subject to YOUR COURNTY'S tarrifs, taxes, impound fees, Customs and Excise,  and other charges.  These charges are imposed by YOUR government, and are NOT included  with the product price or shipping/handling charges that Mandarava charges.  In addition, many countries have restrictions on what items may be imported.

As a United States company, Mandarava does not and cannot include those charges in its shipping fees (this would be illegal).  Also, since every country's laws are different and frequently changing,  we cannot calculate those additional charges for you.   We must, therefore, advise you that you are entirely responsible for any additional charges that your government may impose on your package.    In some cases, customers have received many packages from us without problem--then suddenly, a package will be impounded by their country's Postal Office or Customs Office, resulting in very large additional charges.

PLEASE APPRISE YOURSELF OF YOUR COUNTRY'S LAWS REGARDING THE IMPORT OF PACKAGES.  WE CANNOT REFUND ORDERS THAT ARE IMPOUNDED BY CUSTOMS.

Service Terms and Shipping Information:

All items are normally shipped  US Mail Priority Parcel Post (small items) unless you wish otherwise. Larger items must be shipped UPS Ground (approx. 1-2 wks delivery).   Please be advised that UPS does not deliver to P.O. Boxes. Please email us or call if you have special shipping requirements. Personal and business checks must clear the bank before shipping.  Checks and money orders should be made out to "Mandarava."
For faster service, use your  or  or PayPal.  We will advise you of our PayPal email account name after confirming your order total.

Phone orders: 310-530-1439 (California--Pacific Time).  Please have your items codes and Visa or MasterCharge ready.   We will also need the actual billing where your credit card bill is sent in order to verify the card.  The credit card billing address and the delivery address for your order do not have to be the same.  Your card will not be billed until the actual day we ship the item to you.

If paying by check or money order, please make it out to "Mandarava."   If paying by credit card, be sure to fill out the credit card authorization above and include with your order.  PLEASE INCLUDE YOUR PHONE NUMBER in case we need to discuss your order with you.

Order Form Instructions

1.  Please print out the order form on your printer.

2.  Locate the Item number(s) for the item(s) you wish to purchase from the appropriate price sheet.

3.  Write the item number in the "Item #"  box, for example,"CE106".

4.  Write the description for the item (i.e. "Chakra Energy Pendant-Amethyst,Aqua Aura,Qtz") in the "Description" box. You may shorten this as much as you wish, however, the more description you give the less chance there will be of any error on our part should be have difficulty in reading the returned form.

5.  Enter the number of items you want for this item number in the "QTY" box.  If the item is sold as a set, enter the number of SETS, not the number of individual items.

6.  Enter the price for ONE of the items (if sold individually), or sets (if sold as a set) in the "UNIT PRICE" box.

7.  Multiply the price for ONE of the items (if sold individually) or ONE of the sets (if sold as a set), times the number of items or sets you want total.  Put this amount in the box "Qty. X Unit Price".

8.  "Sub-Total" Box --add up all the items in the "Qty. X Unit Price" column.

9.  "Shipping & Handling" -- use the chart below the order form to obtain the amount to enter in the "Shipping & Handling" box.  Please remember that heavy items such as plaster sculpture or bronzes will have an additional shipping amount under the item on its web page.  Please ADD this amount to the shipping.

10. "Calif. Sales Tax" IF YOU ARE A CALIFORNIA RESIDENT, OR IF THE ORDER IS BEING SHIPPED TO A CALIFORNIA ADDRESS, you must add California Sales Tax.  The NEW 2002 California sales tax for Los Angeles Count is 8 1/4%.  Multiply the "Sub-Total" by .0825 and put into this box.   Out-of-state residents do not need to fill in this box.

11. "TOTAL" --add up "Sub-Total", "Shipping & Handling", "Calif. Sales Tax" and place in this box.

12. Enclose your check or money order with the completed order form and mail to us.  If you have any questions, please email us at sales@mandarava.com or call us at 310-530-1439 (California). Be sure to make out the check or money order to "Mandarava."  Or, fill out the credit card authorization form if you are paying by credit card. Your card will not be billed until the day the item is shipped.
